Job Description

  • Provides complete comprehensive, person centered, patient care for clients, including health examinations and treatment of medical conditions, to include follow-up and any problems detected.
  • Makes appropriate referrals to both internal disciplines of the clinic and/or external agencies for additional services required.
  • Provides clear verbal and written communication to client and staff.
  • Participates in various administrative/ clinical staff meetings.
  • Provider is responsible for 100% compliance with all clinic initiatives.
  • Provider is required to complete documentation with a 100% accuracy in EHR, by the end of the work day 100% of the time. .
  • Provider must correct any coding and billing errors within 24 hrs of notification.
  • Provider is required to respond and complete all client medication and forms requests.
  • Participates with the on-call and emergency coverage schedule as directed by Medical Director.
  • Adheres to the organization's Policies and Procedures.
  • Provides services in accordance with generally acceptable standards of medical practice.
  • Demonstrates courtesy and respect in relationships with professionals within the
  • organization and with professionals in other organizations
  • Returns phone calls and e-mails and coordinates with other community services, sends and responds to releases of confidential information.
  • Provider appropriately expresses information
  • Other duties as assigned by Medical Director and Practice Administrator

Basic Skills

  • Ability to understand and operate necessary computer equipment and software programs. Must be able to receive, interpret and follow verbal and written instructions.
  • Spanish speaking is a plus
  • Excellent written and verbal communication skills
  • Must be self motivated

Experience

  • Experience with ID/DD population is preferred
  • Experience with various Electronic Health Record is required
  • New Graduates welcome

Qualifications

  • Current New York State License and Registration as a Family Nurse Practitioner
  • Current CPR certification
