They are looking for someone with recent homecare field experience. Oasis assessment completed is required.

  • Performs Patient assessments.
  • Accurately assesses patient's appropriateness for home care, learning needs and progress toward goals
  • Develops the plan of care for the patient
  • Accurately completes the OASIS assessment
  • Ensures appropriate utilization of services
  • Maintains visit productivity standards
  • Demonstrates knowledge of insurance reimbursement
  • Completes discharge assessment
  • Performs laptop transfer of data twice daily
  • Transfers OASIS assessment within 24 hours
  • Completes re-visit documentation and transfers back
  • Uses age appropriate teaching methods for patients and families
  • Evaluates teaching plan and revises as necessary
  • Documents patient education tools used
  • Assumes responsibility for Case Management
  • Develops/implements/evaluates plan of care based on evidence-based knowledge
  • Revises care plan as indicated
  • Prioritizes care needs
  • Demonstrates knowledge of home care regulations and insurance reimbursement guideline
  • Shows evidence of interdisciplinary communication
  • Performs HHA supervisions per standards
  • Ensures appropriate utilization of services
  • Follows scheduling process
  • Develops/implements plan of care based on evidence-based knowledge
  • Prioritizes care needs
  • Communicates progress toward goals with IDT
  • Evaluates the plan of care and revises as needed
  • Maintains safe environment for patients and co-worker
  • Follows infection control measures and reports infections
  • Demonstrates knowledge of pharmacology and safe medication management
  • Routinely reconciles medications
  • Maintains current medication orders
  • Participates in Team and IDT meetings
  • Participates in PI meetings and activities/committees as requested.
  • Accepts other assignments as requested
  • Assumes responsibility for case management.
  • Prioritizes schedule to accommodate patient's needs
  • Documents case management activities.
  • Demonstrates knowledge of pharmacology and safe medication management
  • Routinely reconciles medications. Maintain current medication orders.


Knowledge of age-appropriate interventions

Ability to work with individuals of diverse cultures and socioeconomic status


Graduate of an accredited nursing program.; BSN or progress towards BSN preferred

Minimum of 2 years of home care experience in a Medicare certified agency


Hours: Monday- Friday, 8am-4pm. No on call or holiday requirement

(2 openings, Essex County & Union County)

Pay Rate: $46-$48/hr
