Job Description

Health plan dedicated to supporting people with intellectual and other developmental disabilities has several needs for Field Case Managers throughout Westchester County.  

Responsibilities:

• Assess and build a person center plan to ensure the best possible recovery.
• Act as a liaison between clients and providers to ensure care and comfort.
• Facilitate communication and collaborating care between interdisciplinary team.
• Lead role in admission /discharge planning.
• Perform chart review, documentation and monitor compliance with regulation.

Qualifications

• Licensed Registered Nurse (RN)
• Home care or recent hospital experience in Critical Care or the Emergency Room 
• MR/DD experience preferred
